WHAT SHOULD I BRING ALONG TO MY APPOINTMENT?

• Medical Aid card
• ID
• Referral letter
• A list of medications you are on, whether they are prescription or over the counter, including the doses you are taking. If unsure, bring the medication along, in its box.
• Any results of blood tests or scans you may have, especially if you had them performed outside of the Eastern Cape, or in a public hospital/clinic, as these are more difficult (but not impossible) to access.
